Fully funded by the BC Ministry of Health Patients as a Partners Program. All of their materials and workshops are FREE and available for all adult BC Residents.

Self-Study Tool Kit for Active Living
- For the independent approach, one-time mailing of a kit.
- Offered in Chronic Conditions, Chronic Pain, and Diabetes.
- Kit includes: “Living a Healthy Life” book, booklet and MP3 codes.
Tool Kit for Active Living plus Calls
- A 6-week program where facilitators guide a maximum of 6 participants. Participants attend this workshop on a weekly 45-minute telephone conference call.
- Available for Chronic Conditions, Chronic Pain, and Diabetes.
- Materials include: “Living a Healthy Life” book, booklet and MP3 codes.
Virtual Self-Management Programs
- A 6-week program where facilitators guide a maximum of 12 participants in a weekly 2.5 hour Zoom®workshop.
- Available for Chronic Conditions, Chronic Pain, Diabetes & Cancer: Thriving and Surviving.
- Materials include: “Living a Healthy Life” book and a Personal Health Record booklet.
