Nestled in the heart of British Columbia’s West Kootenay region, on the traditional and unceded territories of the Sinixt, Syilx, and Ktunaxa Nations, West Kootenay Nav-CARE is a compassionate initiative dedicated to improving the quality of life for seniors and adults living with chronic illness. By matching trained volunteer navigators with individuals in need, Nav-CARE offers personalized support that addresses the emotional, social, and practical challenges of aging and illness. Our mission is simple yet profound: to empower individuals to live well despite the complexities of aging and health issues. With a focus on connection, advocacy, and community, we aim to combat the isolation and uncertainty often faced by seniors, individuals with disabilities or declining health, and their families.

Who We Are
West Kootenay Nav-CARE operates across the region, serving communities including Nelson, Kaslo, Nakusp, Slocan, Castlegar, Trail, and Salmo. Our program thrives on the dedication of volunteer navigators—caring individuals who offer their time, energy, and compassion to make a tangible difference in the lives of others. Behind the scenes, our coordinators play a pivotal role in recruiting, training, and supporting these volunteers. Coordinators ensure that each participant is matched with the right navigator to meet their unique needs and are available to help volunteers with any complex situations that arise. This structured yet deeply personal approach allows us to provide truly impactful support to our community.

Advocacy and Empowerment Navigating healthcare systems can be an overwhelming experience, especially for those dealing with chronic illness. Our navigators serve as advocates, helping participants understand their options and access necessary resources. By empowering participants to make informed decisions, we help restore a sense of control and confidence in their lives.

Practical SupportThe day-to-day challenges of aging or illness can often feel insurmountable. Whether it’s arranging transportation to medical appointments, helping with grocery shopping, or connecting participants with local meal programs, our navigators provide practical support that makes life more manageable.

Holistic CareAt Nav-CARE, we understand that true well-being extends beyond physical health. We address the whole person—mind, body, and spirit—by connecting participants with grief support, wellness programs, or activities that bring joy and fulfillment.
